A Clinical Analysis of 430 Cases of Panperitonitis



Abstract
We have a clinical review of 430 patients with acute generalized peritonitis who were treated at the department of Chung Ang university hospital from June, 1968 to June, 1983. The result obtained are as follows
1) During the period, the patients with acute abdomen who underwent laparotomy were 3,104 cases, of which 430 cases were generalized peritonitis and its percentage were about 14%.
2) Among the 430 cases, acute perforated appendicitis were 143(33.3%), peptic ulcer perforation 128(29.8%), traumatic visceral perforation 68(15.8%), and typhoid perforation `26(6.0%).
3) The sex ratio was 2.5 : 1(male: female) and the majority of patients were 31 to 40 years old, 92 case(21.4%)
4) The time elapse between the onset of generalized peritonitis and hospital admission was markedly prolonged in cases of typhoid perforation as compared with those of both peptic ulcer and traumatic visceral perforation.
5) The most frequent postoperative complication was wound infection(rate 31.6%). Typhoid perforation showed the highest incidence of wound infection(rate 44%). The pulmonary complication and sub phrenic abscess were frequent in the case of peptic ulcer perforation.
6) The majority rate was 3.9% and typhoid perforation and peptic ulcer perforation showed the highest rate. Nine patients were expired within postoperative 7th day and 8 cases expired due to sepsis.
